We will begin seeing patients in the Springfield location on Wednesday, June 10th. You must have a scheduled and confirmed appointment to enter the office. Due to space and safety restrictions, we can not allow anyone to enter the office without an appointment.
Only the patient will be allowed entrance unless a caregiver is required or a child requires a parent (and in those instances only one additional person will be allowed to enter the office).
Temperature and pulse oximetry will be taken – anyone with a temperature over 100o F or a pulse o2 of less than 95 will be required to reschedule.
A staff member will contact you prior to your in-office testing to begin the examination to reduce your time in the office.
Dr. Maizel will contact you by telemedicine after your visit to review your test results and answer your questions; reducing your time in the office.
If you have been in contact with any person who is currently confirmed with COVID-19
If you have been in contact with any person who is currently being quarantined for COVID-19
If you have a fever (or are taking a fever-reducing medicine) or have any of the following symptoms:
Loss of smell or loss of taste, Dry cough, Difficulty with breathing, Confusion, Muscle fatigue, Sore throat, GI symptoms or diarrhea, extreme fatigue, Headache, Chills, Conjunctivitis, Rashes on your toes
WE WILL RESCHEDULE YOUR APPOINTMENT IF YOU ARE EXPERIENCING
ANY OF THE ABOVE SYMPTOMS.

We are also offering telemedicine appointments for anyone who does not wish to come into the office at the present time.
